Decontamination statement
Because of legal regulations and for the safety of our employees and operating
equipment, we need the "decontamination statement" completely filled out and
signed", before your order can be handled.
All items being returned must be packaged separately. Fill out a separate
decontamination statement for each item being returned.
Attach the decontamination statement to the outside of the packaging so that it
is easily visible.
